Life and Health Insurance Claims

If your insurance policy is with Sovereign we advise you to contact them directly to ensure your Claim is handled quickly and effectively.
 
How to make a life insurance claim

 
Notify Sovereign
Call their Customer Relationship Team on 0800 500 108, they are there to help at this stressful time.
 
 
Requirements
To assess your life insurance claim Sovereign will need:
  • The original policy document or Lost Policy Declaration form
  • The original copies of birth certificate or passport or certified copies of either
  • The original death certificate stating the cause of death and coroner’s report (if applicable). A death certificate awaiting coroner’s report is not sufficient.
If the policy was soley owned by the deceased we also require:
  • Probate or Letters of Administration (only if the value of the estate is greater than $11,000)
  • Statutory declaration and/or copy of the will (only if the value of the estate is $11,000 or less).
 
Decision time
When all requirements have been met and the claim accepted, a claim discharge form is sent to be signed by the owner(s) of the policy; OR the executor of the estate.


Payment

Payment will be made by direct credit upon receipt of the signed Claim Discharge form, along with an encoded bank deposit slip.
 
 
How to make a health insurance claim

Almost everyone needs to claim on health insurance at one time or another, so Sovereign have made it easy.
 
You can apply for approval in advance, so that you can be confident treatment costs will be covered. Pre-approval means that Sovereign can arrange to pay your providers directly.
 
To apply for pre-approval, please call the Customer Relationship Team on 0800 500 108. You should contact us at least five working days before your admission or procedure.
 

Notify Sovereign
They will require information to support your claim, which may include:
  • A completed Health Insurance Claim form and;
  • A medical certificate, completed by your GP OR a copy of the GP referral letter OR a copy of the specialist report, containing the following:
  • Start date of symptoms
  • History of condition
  • Treatment received.
If you have already had the procedure or test we can retrospectively assess the claim.

Note: Sovereign does not cover the cost of obtaining a medical certificate from your GP.


Decision time
If prior approval is given, we guarantee to pay all relevant invoices, less any excess you have selected, or other offset provision. This approval is valid for six months.

You need to complete a claim form and send it to us together with the registered medical practitioner's referral letter and/or medical certificate and all of the original receipts and itemised invoices.


Payment
Payment will be made directly to the treatment provider unless original receipts are received. Reimbursements for receipts are made by direct credit. Page three of the claim form has payment information where you can list your account details for reimbursement.
